Do you have a health condition?\u00a0 Does someone in your family take medication on a regular basis?\u00a0 If so, you may want to carry a medication card in your wallet.<\/p>\n

It\u2019s important to know the names and dosage amounts of all medication.\u00a0 This means prescriptions and over the counter medications.\u00a0 In a medical emergency, the doctors will need to know this information.<\/p>\n

Below you will find a blank Medication Wallet Card.\u00a0 This card was designed to fit in your wallet, and is the size of a credit card.<\/p>\n

Step 1<\/strong>: Print the free printable Medication Wallet Card below.<\/p>\n

Step 2<\/strong>: Enter your information.<\/p>\n

Include the full name of the medicine as well as the dosage amount.\u00a0 It may also be helpful to write down the time you normally take this medicine.<\/p>\n

Next, fill out your Emergency Contact Information.<\/p>\n

Step 3:<\/strong> Cut to size.\u00a0 When finished, it should be the size of a credit card.<\/p>\n

Step 4:<\/strong> For more stability, glue the two pieces onto heavier paper and keep in your wallet.<\/p>\n
